Please provide the following information:
·      Your name, as it read at time of graduation.
·      Birth date.
·      Year of graduation.
·      A legible copy of a picture ID with your signature.
·      State if you require an unofficial or sealed official transcript.
       Faxed or emailed transcripts are unofficial. Official transcripts must be mailed.
·      If you need the copy mailed, please provide us with your complete mailing address.
We will be unable to process your request without all the above information included.
