7th Grade Immunization Requirements for 2024-2025

All students entering the 7th grade in the 2024-2025 school year must receive a Tdap (Pertussis/Whooping Cough) booster and second Varicella (Chicken pox) vaccine as well as all other required immunizations before entering school on August 7, 2024. Until all required immunizations are complete per state law, students will NOT be allowed to:

• Receive a schedule for their 7th grade classes
• Attend school in-person
• Participate in school sponsored activities

Proof of the vaccine must include the following:

• Student’s name
• Date of Birth
• Vaccine name
• Date vaccine administered
• Name of the clinic, office, or doctor administering the vaccine

Please give them to the Health Assistant at your child’s school or you may email the immunization records in PDF or JPG file only to the Health Assistant. Call your child’s school for the email address.
